Please bring the following items to your appointment:
- Photo ID & Insurance Card(s)
- List of all current medications, including vitamins and supplements
- Your health history — conditions, procedures/surgeries, allergies, etc.
- List of any questions or concerns about your eyes or vision
In addition, we always suggest that you bring someone to drive you home since your eyes will remain dilated for several hours. Sunglasses may help reduce glare. If you need some, please ask at the front desk before you leave. If needed, our receptionist can also help you arrange for transportation.

Please remember to bring the completed New Patient Registration Packet with you when you come in for your appointment or fill out the online form below to submit directly to the practice.
Your Visit
Arrival
When you arrive at our office for your appointment, you will be greeted by our receptionist who will help check you in and make sure we have all of your current contact, health and insurance information. When you make your appointment, you may be asked to arrive 15 minutes early to complete the registration process.
Evaluation and Diagnosis
Our medical team can often evaluate, test, diagnose and treat you all on the same day. As a result, an initial comprehensive evaluation, with or without treatment, could last approximately two hours.
On each visit, we will perform a vision test and a measurement of your intraocular pressure, followed by dilation of your eyes. After a wait to allow your pupils to dilate adequately, Dr. Jackson will examine you.
Treatment
If treatment of your eye problem is indicated, you and Dr. Jackson will decide the most appropriate course of action. Some types of treatment can be performed in our office at the time of your visit. Others may have to be scheduled at a local outpatient surgery center or hospital.
Follow-up Visits
If you are coming to our office for a follow-up appointment, your visit can last up to two hours, depending on the specific testing and treatment you need. For your safety, we recommend that you have a friend or family member drive you home from any appointment involving treatment or procedure.
